Divisional Secretariat - Matale - Payment of Pensions

After getting the pension file from the institution where the last worked the pensioner, we have to produce the following documents to prepare the pension.

01.		Retired Identity Card
02.		Residence Certificate obtained from the Grama Niladhari relevant to the residence.
03.		A photocopy of the front page of the bank book
04.		Retirement declaration

When our office receives the death gratuity file, informing our office to terminate the civil pension for the deceased,
Please hand over a requesting letter of widows' orphan's to us with a photocopy of the death certificate and a widows' orphan's number
After receiving a letter from the Department of Widows or Orphans of Department of pensions, the following documents should be forwarded


01.		Original Certificate of marriage
02.		An affidavit if there is a difference in name
03.		Original copy of death certificate
04.		A copy of the pensionable salary sheet
05.		Birth certificate of children under 21 years
06.		3 photographs (3 ¼, 2 ¼ size) with Grama Niladari recommendations should be provided
07.		bring me your National Identity Card as well
08.		Submit the small form to the Grama Niladari
09.		A letter obtained from the relevant divisional secretariat that the pension is terminated
10.		Birth certificate of Widow / Widow's

Also, Please get followings from our office

01.		The letter should be submitted to the grama niladari
02.		No. 15 form of Pensioners' Fund

After we receive the Death gratuity file,
when a letter has been sent to the office from the Department of Pensions, the widow orphan should come to our office with the following documents.

01.		Birth certificate of the orphan'
02.		Originals death certificates of mother and father
03.		Marriage certificate mother and father
04.		If children less than 18 years should come with a guardian above 21 year, 5 / 1, 6,7,8 applications should be submitted
