Registration of a new business
Application form for submitting for the registration of a new business. Please submit the following documents completely.

Application form for registering a new business (affixing a stamp of Rs. 5.00, but when a Justice of the Peace signs a partnership business application, a stamp of Rs. 100/= should be affixed).

Application letter.

A photocopy of the trade license issued for that year by the relevant local authority (Municipal Council/Pradeshiya Sabha).

Certified copies by the Grama Niladhari of the deed related to the business premises and the original folio (deed / absolute/sinna-kara).

If the land belongs to another owner, a consent letter signed by the owner in front of the Grama Niladhari expressing consent to register the business name.

A certificate confirming qualifications must be submitted when carrying on a business (Ayurvedic).

Public Health Inspector's report (for selling food items, canteens, hotels, beauty salons).

A photograph showing how the name board of the business place is displayed.

Environmental officer's report as required.

A photocopy of the applicant's national identity card.

Grama Niladhari report related to the business place.

If it is a factory or a workplace, registration from the District Factories Inspecting Engineer's Office.

Approval of the Range Forest Office if it is related to timber.
(All photocopies submitted must be copies certified by the Grama Niladhari.)

Issuing income certificates
| 01. | Obtaining the recommendation of the Grama Niladhari after completing the income certificate application. |
| 02. | Submitting documents confirming income along with the recommendation of the Grama Niladhari. Submitting with the request letter of the applicant to the Divisional Secretariat. |
| 03. | After obtaining the approval of the Divisional Secretary, the applicant should pay 1.15% of the annual income to the Divisional Secretariat. |
| 04. | After the payment, the income certificate is issued and the signature is obtained that it has been accepted. |

Issuing value certificate
| 01. | Obtaining the recommendation of the Grama Niladhari after completing the value certificate application. |
| 02. | Submitting the application to the Divisional Secretariat along with the request letter, deed and its copy, land deed, transcripts obtained from the Pradeshiya Sabha and the building plan approved by the Pradeshiya Sabha if it is a building. |
| 03. | After obtaining the approval of the Divisional Secretary, the applicant should pay 1.15% of the annual income to the Divisional Secretariat. |
| 04. | After the payment, the value certificate is issued and the signature is obtained that it has been accepted. |
